South Hilyard Clinic has a new name, new look, and now a new website! In case you missed the details about these changes, check out our Fall Newsletter.
Our new look has been designed for ease of information gathering, helpful to your continued care, and optimized for use on any mobile device so that you can keep in touch with us while on the go. We’ve made it easy for you to access your online health records through our patient portal, get in touch with our office to make an in-person or Televisit appointment, and easy to download any necessary medical forms to print and fill out before you come to your appointment. We will also be offering easy access to pay your bill online very soon – stay tuned for that addition to our new site!
Some of the essential sections of the new website are:
- We will post updates on things happening in our office, such as our new provider, Laurel Merz, FNP, joining us in March, and healthcare updates that are important to be aware of (locally and globally) that keep you living healthily.
- Our Patient Information page has everything you need in one place from new adult and pediatric patient information, downloadable forms, billing information, how to access a Televist appointment, resources, and our clinic’s programs. We want you to be prepared for your visit and to host easily-accessible resources for you to learn more about topics you and your provider may have discussed during your visit.
- We want to provide you with a personal healthcare experience. Learn about our team of providers to see who would best fit your specific healthcare needs.
- We’re always looking to grow our team of providers. It’s important to Valley Med that our team members are dedicated and determined to serve our patients, generate long-term relationships, and provide high-quality healthcare to our patients.
